This App is owned by Patient Plus ERP. It is powered by Patient Plus (Sismatix), which acts only as a technology provider.

2. Nature of Service

This App is a platform connecting patients with the clinic. The clinic is solely responsible for:

• • Appointments
• • Pricing

Sismatix does not provide medical services.

3. User Acceptance

By using the App, you accept these Terms as a binding agreement.

4. User Obligations

You must:

5. Medical Disclaimer

This App does not replace professional medical advice.

6. Appointments

The clinic may confirm, cancel, or reschedule appointments.

Payments (if available) are processed on behalf of the clinic and are subject to clinic policies.

8. Limitation of Liability

To the extent permitted by law: The clinic is responsible for medical services; Sismatix is responsible only for the system. Sismatix is not liable for medical outcomes, appointment issues, or indirect damages.

9. Availability

We do not guarantee uninterrupted service.

The clinic may suspend accounts in case of misuse.
